This UK 4th press version comes in a generic black die-cut cover with or without trimmed corners. 

It was also issued in the following sleeves: 
Two Bodies sleeve,
Who Chance Blink ZTT sleeve,
U.S. Chequerboard sleeve (these were returned sleeves from Island U.S.A.)

It has a speckled, pale green A-side label showing "Relax (Sex Mix)", but the record actually plays most common mix: the 7:20 "Relax (U.S. Mix)", which also appeared on the U.S. 12" (Long Mix). This version can be identified by the stamped matrix 12 ZTAS 1 A - 5U on the A-side.
